Falls Screening:
Meet one-on-one with a trained facilitator to discuss your fall concerns and explore individualized solutions. The initial meeting takes approximately an hour and is completed in-person. Appointments are every 2nd Wednesday of each month at 2:30pm.
Memory Screening:
Meet one-on-one with a trained facilitator to discuss memory concerns and explore individualized solutions. Appointments are every 2nd Thursday of each month at 11:30 am and 12:00 pm.
